Patient Safety Assessment Manual

Patient Safety Assessment Manual (PSAM) contains a set of standards with requirements that are needed for the establishment of a patient safety program at the hospital level.

It provides a framework that enables hospitals to assess patient care from a patient safety perspective, build the capacity of staff in patient safety, and involve consumers in improving health care safety.

This manual provides the necessary tools for professional associations, regulatory, accrediting or oversight bodies and ministries of health, to improve patient safety.

The manual has two sections:

  1. Patient safety standards; and
  2. Patient safety friendly hospital assessment tools.

Patient Safety Standards:

It involves 5 domains

A. Leadership and management

B. Patient and public involvement

C. Safe evidence-based clinical practice

D. Safe environment

E. Lifelong learning.

Patient safety friendly hospital assessment tools:

A set of tools to facilitate the assessment process, including a suggested agenda for the assessment visit,  a complete list of all documents required from the hospital, an observation checklist and interview questionnaires collated by the interviewee (For example, all the standards that can be determined by asking the infection control specialist will be found collated in the interview form with the infection control specialist).It also includes guidelines for the evaluator including documents to be reviewed for each standard, relevant interviews, an observation guide, and scoring guidelines.

  • Join the meeting well in advance to ensure video and audio are tested
  • Use a very stable ADSL internet connection
  • Avoid any background distraction
  • Participants response will not be available as all audience will be asked to stop video and audio to reduce the use of bandwidth
  • Participants can pin the lecturer to see in full screen
  • You need to remain in the room all 5 days 3 hours only during break the time you are allowed to leave
  • Only when it is your time to speak switch on your audio and video
  • The course will have Host (the speaker) and co-host to ensure to ensure all the technical issues are answered and put all participants’ video and voice and set off.
  • This course will be recorded and shared to all at the end of the program by the lecturer
  • To grant a certificate all must attend 5 days and 3 hours/ day they will be daily attendance taken by the organizer.
